CeutiCare engages with an acute care provider (hospital/health system) to provide C-MTM services to its primary care network. We locate pharmacists on-site at a physician’s office that is affiliated with or supported by an acute care provider to improve the health of chronic care patients such as diabetics, and reduce the overall cost of treatment.
Our pharmacists work collaboratively with the physician. The pharmacist monitors chronic care patients closely, meets with them frequently, and discusses their medication needs with the physician and the patient, recommending medication therapy for each patient for the physician’s consideration. We provide education, encourage lifestyle changes and recommend adjustments to medications to help produce better outcomes.
Our pharmacists can also meet with pharmaceutical sales representatives to help minimize the time that physicians might otherwise spend speaking with them.
We use proprietary decision-enabling software consisting of evidence-based decision-support algorithms that take into account individual differences in patients including all related aspects of co-morbid conditions e.g. CAD, PAD, thyroid deficiency, renal function, liver function, CV disease and insulin resistance.
